More options means more confusion. Here’s a practical framework for evaluating TRT clinics so you choose care quality over marketing.

Questions to Ask Every Clinic

1. Who designs my protocol?

Is a physician (MD/DO) reviewing your labs and designing your treatment? Or is it a nurse practitioner following a template? For straightforward cases, either may work. For anything complex — cardiovascular risk, fertility concerns, combination therapies — physician oversight matters significantly.

2. What labs do you order before starting?

A complete pre-treatment panel should include total and free testosterone, SHBG, estradiol, LH, FSH, PSA, CBC, and comprehensive metabolic panel at minimum. Any clinic that starts TRT based on total testosterone alone is cutting corners.

3. How often do you monitor after starting?

Standard of care: follow-up labs at 6 weeks, then every 3–6 months. Clinics that check you once a year are not providing adequate safety monitoring. Ask specifically about hematocrit, estradiol, and PSA monitoring frequency.

4. What’s included in the monthly fee?

Some clinics advertise low monthly prices that exclude lab work, consultation fees, and ancillary medications. Get total cost transparency before signing up. A clinic at $99/month that bills labs separately may cost more than one at $150/month with comprehensive monitoring included or billed to your insurance.

5. Can you adjust my dose if I’m not responding optimally?

Some high-volume clinics run standardized protocols with limited flexibility. If your levels don’t respond as expected, do you get a physician who adjusts your protocol — or are you stuck at “this is our standard dose”?

  • Prescribing testosterone based on symptoms alone, without confirming labs
  • No PSA check for men over 40, before starting (prostate safety baseline)
  • Annual (or less frequent) blood monitoring
